Steps to Describe What Happened
Identify the incident

State whether the issue is ongoing
Include any steps you have already taken
Provide your preferred contact method (may be optional)
State clearly what you observed, experienced, or discovered.
Provide the date and time

Indicate when the incident occurred or when you become aware of it.
Describe the location

Specify where the event took place (physical location, agency, program, platform, or any other place or location).
Explain who was involved

Detail the actions or behavior
Describe what happened in chronological order. Focus on facts, not assumptions.
Attach or reference evidence
Include documents, screenshots, communications, transactions records, or any supporting material.
Describe how the incident affects you, others, the agency, the resources or the public.
Explain the impact or risk
Indicate if the behavior continues or if it was a one‑time event.
Mention whether you reported it elsewhere, contacted an agency, or attempted resolution.
